HMO & PPO Eliminated

You should have received your new Blue Cross Blue Shield card for Anthem Transitional Care due to the elimination of both HMO and PPO coverage in Ohio. GM sent you a notice in early December about the changes. It is important that you read the changes so you do not cost yourself additional out of pocket monies.
